To respect the time of our providers and other clients, we kindly ask that you review the following appointment policies for

aesthetic and beauty services.

Relevant Services

This policy includes but is not limited to beauty and aesthetic treatments, including facials, microneedling, neurotoxin

injections, dermal fillers, laser treatments and chemical peels.

Card on File

A valid credit card must be on file in order to reserve an appointment for aesthetic and beauty services at Midwest Medical.

The card will only be charged if a cancellation or no-show fee applies.

Cancellation & Re-scheduling

We require at least 24 hours’ notice if you need to cancel or reschedule your appointment. Appointments cancelled with less

than 24 hours’ notice will incur a $25 late cancellation fee.

No Shows

If a client does not arrive for their scheduled appointment and no notice is provided, a $25 late cancellation fee will be

charged to the card on file.

Late Arrivals

Clients arriving 10–15 minutes late may have their appointment shortened to accommodate the schedule. If there is not

enough time to complete the service, the appointment may need to be rescheduled.

Packages or Treatment Series

Missed appointments for services purchased as part of a package or treatment series may incur a $25 missed appointment

fee.

Repeat Missed Appointments

Clients with repeated late cancellations or missed appointments may be required to pre-pay for future appointments at the

time of booking.
