📄 Membership Cancellation Policy – Midwest Medical Weight Loss & Aesthetics

We understand that circumstances change. Please review our cancellation policy below to ensure a smooth process.

🔁 Cancellation Terms

Notice Requirement: Members must provide a 30-day written notice prior to cancellation.
Minimum Commitment: Memberships have a 6-month minimum term before cancellation is allowed.
Cancellation Form: A cancellation request must be submitted in writing or via our online form (available at Midwest Medical Weight Loss & Aesthetics.

💳 Final Charges

You will be billed one final monthly payment after cancellation notice (if within the 30-day period).
Any unused credits or treatments must be used within 60 days of cancellation.
No refunds only credits

❌ Early Termination Fee

✔️ If membership is canceled prior to the 6-month minimum, a $200 early termination
fee applies (or prorated depending on the membership type).

⚠️ Reactivation

✔️ Former members may reactivate their membership with a $50 reactivation fee within 6
months of cancellation.

✔️ After 6 months, rejoining requires a new enrollment
